Cracking the Code on Healthcare - June 23, 2015

You are cordially invited to join us for our first Cracking the Code on Healthcare SPOTLIGHT event. This event is specifically planned for nursing and long term care providers. The event will be held June 23, 2015 at Irondequoit Country Club from 7:30 AM-11:00 AM.

The event title is Improving Care Transitions.

Better coordination of patient transfers among care sites and the community has shown to improve patient outcomes and satisfaction as well as save money for the individual and for the community. There are many root causes of poor care coordination. During the key note presentations and panel discussion we will provide the latest thinking on a number of topics including:

  • Transition of care models
  • Implication and solutions when communicating through different computer systems.
  • Implications of Medicare, Medicaid and commercial payment policies
  • The role of primary care physicians

The event will include these esteemed speakers:

Mary D. Naylor, PhD, FAAN, RN, is the Marian S. Ware Professor in Gerontology and Director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing. For the past two decades, Dr. Naylor has led an interdisciplinary program of research designed to improve the quality of care, decrease unnecessary hospitalizations, and reduce health care costs for vulnerable community-based elders. Dr. Naylor's talk is titled The Transitional Care Model: A Journey From Evidence to Impact.

Denise L. Fletcher, Vice President and Chief Innovation Officer at Xerox Corporation is an accomplished executive leader with 25+ years of proven expertise in financial and healthcare services and significant experience in consulting environments. She is responsible for leading innovation within the Healthcare Payer and Pharma (HPP) Business Group at Xerox. In this role she has established an innovation framework in concert with the Kellogg School at Northwestern that delivers repeatable, sustainable top line growth projects in support of customer and industry needs. Ms. Fletcher's talk is titled Work Processes and Data Directed at Improving Transitions of Care.

The event will feature a Panel Discussion facilitated by Howard Beckman, MD, Clinical Professor of Medicine, Family Medicine, and Public Health Sciences at the University of Rochester School of Medicine and Dentistry.  The Panel includes:
